The last time we reported on Zertane we did not know the active ingredient used. We recently discovered that Zertane’s active ingredient is tramadol hydrocloride which is used in Ryzolt and Ultram. These drugs are opioid analgesics. We do not know much more about the drug but would like to hear from anyone who has noticed any effect of analgesics on premature ejaculation.

Ampio evaluated 604 patients in a study comparing the latency results. They found a four time increase in latency. Among men with a median IELT of 1 minute or less at baseline, the results were even more significant. Mean IELT increases were 5.2-fold for the lower dose of Zertane and 6-fold for the higher dose. “We are aggressively pursuing regulatory approval for Zertane in Europe and are in discussions with potential distribution partners in Europe and Asia,” comments Don Wingerter, CEO.

The drug has been shown to be more effective than Priligy but neither is available in the US.

Ampio Pharmaceutical has announced that the results of it’s latest clinical trials for the drug Zertane currently in it’s final phase of trials, has exceeded expectations.

We don’t know when the drug will be launched but we know that they are targeting the European market first. It is also not clear what the mechanism of action of Zertane is but according to Ampio’s website:

The active ingredient in Zertane™ has multiple mechanisms of actions that can delay ejaculation. This drug also has an excellent safety record established during 30 years of human use. These unique pharmaceutical qualities, exceptional human safety record, and a distinctive non-standard dosage not available in generic form, differentiate Zertane™ from other treatments for premature ejaculation.

Our guess is that it is not an SSRI drug like Priligy but we’ll keep you posted.

Finally some good and consoling news for premature ejactulators. According to an article on Scientific American there are evolutionary advantages to ejaculating quickly.

The author compares the mating habits of human beings to other rapid—and not-so-rapid—ejaculators in the primate family, noting that the faster a primate species is in the coital realm, the less aggressive it is when it comes to mating-related behaviors. He calls this the “slow speed – high aggressiveness hypothesis.” For example, male rhesus macaque monkeys often engage in marathon mounting sessions, where sex with a female can be drawn out for over an hour at a time (including many breaks and therefore non-continuous thrusting). That may sound great, but libidinous anthropomorphizers beware: macaque sex is a chaotic and violent affair, largely because the duration of the act often draws hostile attention from other competitive males. By contrast, primate species whose males evolved to ejaculate rapidly would have largely avoided such internecine violence, or at least minimized it to a considerable degree.

According to a new study, fatter men last longer in bed, while leaner men are more prone to premature ejaculation. The study came from Erciyes University in Kayseri, Turkey and found that men with excess body fat develop more female sex hormones that influence their sexual performance.

Men with high fat levels were found to have higher levels of the female sex hormone oestradiol, which disrupts the chemical balance in their body, making them last longer during sex.

The survey’s results found fat men could last an average of 7.3 minutes during love making, while others only lasted 1.8 minutes.

To find the results, researchers spent a year recording the body mass index (BMI) of more than 100 patients referred for specialist treatment.
They compared these results with 100 other male patients who lasted longer during sex.

Results concluded that the men needing treatment for premature ejaculation had lower BMI scores, meaning they were fitter.

“We found that premature ejaculators were leaner,” the report stated.

Other research has already found that the level of estradiol is low in men who suffer from Premature ejaculation. This seems to corroborate the study.

The first U.S. trials of a spray-on anesthetic for the penis showed that it increased the time to ejaculation nearly five-fold, providing the first good solution for premature ejaculation, researchers reported today. The findings are very similar to those obtained in a European study released in April.

Premature ejaculation is generally defined as ejaculation that occurs within a minute after insertion of the penis into the vagina, causing distress to both parties. The condition is thought to affect as many as one in three U.S. men ages 18 to 59, about twice as many as those who suffer from erectile dysfunction. Some antidepressant-like drugs, such as dapoxetine, have been approved in a few countries to treat the condition, but the Food and Drug Administration rejected it because of long-term side effects. Some physicians prescribe anesthetic creams like EMLA cream for off-label use to delay ejaculation, but such creams require 45 minutes to work and the man must use a condom to prevent the anesthetic from numbing the woman.

The spray, dubbed PSD502, contains the anesthetics lidocaine and prilocaine. But it doesn’t deaden feeling, thanks to an ingredient in the spray that allows it to rapidly penetrate the skin, says Ira Sharlip, MD, clinical professor of urology a the University of California, San Francisco.

“It rarely causes a decrease in sensation in the penis because it is absorbed so quickly,” Sharlip tells WebMD. “In men using the product, the incidence of penile anesthesia is 1% or 2%.”
Plethora Solutions of London and Sciele Pharma Inc. of Atlanta have developed a spray anesthetic, called PSD502 or Tempe, that contains lidocaine and prilocaine dispensed by a metered aerosol. It is applied five minutes before intercourse, and it selectively numbs the head of the penis.

The new trial, led by Dr. Michael G. Wyllie of Plethora enrolled 256 patients at 38 centers in the U.S., Canada and Poland. Their average duration of intercourse was less than 0.6 minutes. Two-thirds of the participants used PSD502; the rest were given a placebo. Wyllie reported today at a San Diego meeting of the Sexual Medicine Society of North America that men receiving the drug increased their intercourse time to an average of 2.6 minutes, compared with an increase to only 0.8 minutes in the placebo group. The benefits persisted for the three months of the study and both men and women reported greater satisfaction with their sexual experiences. No significant adverse effects were observed.

The company expects to apply for FDA approval of the drug in the first quarter of 2010, Sciele Chief Executive Patrick Forteau said.

According to a new study, many men may suffer from PE because of chronic prostatitis, which may be treated with an antibiotic.

Premature ejaculation (PE) is the most common sexual problem, and chronic prostatitis is an important cause of PE.

The aim of this study was to determine which clinical parameters predict successful outcomes following treatment of men with PE and chronic prostatitis (category II and IIIa).

Main Outcome Measure: Change in intravaginal ejaculatory latency time (IELT) and its relation to different clinical parameters.

This study included 210 heterosexual men with PE and inflammatory prostatitis. PE was found to be acquired in 155 men (A-PE) and lifelong in 55 (LL-PE). All participants were asked to complete the National Institutes of Health-Chronic Prostatitis Symptom Index (NIH-CPSI). Sequential microbiologic specimens were obtained. Antibiotics were given to 184 men for 4 weeks, guided by sensitivity tests. Twenty-six men refused or did not comply with the antimicrobial therapy and were used as the untreated group. Clinical reevaluation was conducted after 28 days.

Two of the 26 men (7.7%) from the untreated group experienced an increase in their ejaculatory latency compared with 109 of the184 men (59.0%) who received antimicrobial therapy (P = 0.0001). After treatment, 90 of 155 men (58.0%) with A-PE reported ILET > 2 minutes compared with 21 of 55 men (38.2%) with LL-PE (P = 0.012). Based on a receiver operating characteristic curve, antimicrobial therapy is most effective if there are >/=19 pus cells per high-power field (HPF) in the expressed prostatic secretion (EPS) analysis, with a sensitivity of 85.6% and a specificity of 70.7% (area under the curve 0.783, 95% CI 0.716-0.850). Other clinical parameters were not useful in predicting outcomes.

Antimicrobial therapy is useful in the treatment of PE associated with inflammatory prostatitis. The treatment is most effective in men with A-PE and when there are >/=19 pus cells per HPF in an EPS analysis. Zohdy W.

One major contributing factor to premature ejaculation is friction. Friction during sex comes about from the back and forth, in and out movement that is applied during intercourse. This continued thrusting and gyrating heightens friction and also increases the chances of early ejaculation.

To reduce friction and delay ejaculation there are certain sex positions that needs to be avoided. In addition to those, there are some positions that can be utilized to greatly reduce the occurrence of ejaculation

Any position that requires the majority of the effort to be done by the male will increase the chances of him ejaculating too soon. Positions where he is on top, missionary and ones where he enters from behind, calls for extra pushing, thrusting and gyrating from him. All this movement will increase friction, heighten his excitement and so greatly increase the possibility of premature ejaculation. Avoiding these positions will help to prevent early ejaculation.

Here are the positions that will actually assist in preventing ejaculation and prolonging pleasure for both partners. Positions which call for the female to straddle or sit atop the male are good. These require almost no work from the male. Pleasure is derived from the female partner’s movements and she can slow it down to further assist in the delay of ejaculation..

The female sits on top facing or with her back turned towards the man. He either sits or lays on his back relaxed and she does all the work. With this position there is less pressure on the male to perform and because there is less movement by him friction decreases and the chance of early ejaculation is reduced.

A man’s control over his ejaculation can be greatly increased by the sexual position the couple engages in. For example, the missionary position makes it difficult for a man to control his ejaculation. Coitus with the man on his back gives the control of the sexual act to the woman and lets the man relax. He is then in a better position to guide his partner.

A new drug is in clinical trials and may offer hope for men suffering from premature ejaculation. A press release today revealed (pre-clinical) trials for a drug called NHo2D. According to the company (NeuroHealing Pharmaceuticals), it is a novel mechanism. We are not sure how novel or where it is novel and if the drug relies on SSRI’s like the ones found in anti-depressants or if the mechanism is completely new.

NeuroHealing Pharmaceuticals, a private, clinical stage company developing innovative drug treatments for specialty indications based on repositioning neurologically active compounds, announces the formation of a Medical Advisory Board (MAB) to assist the company in the clinical development of NH02D, its novel mechanism of action, drug candidate for the treatment of premature ejaculation (PE).

NH02D is a neurologically active, orally administered drug that is being developed for on-demand treatment of premature ejaculation. During testing of NH02D in healthy volunteers for another indication, the drug was unexpectedly reported to delay ejaculation during sexual activity. Subsequently, the effectiveness of NH02D has been demonstrated in a validated animal model of premature ejaculation in collaboration with Dr. Lesley Marson at the University of North Carolina.

Premature ejaculation is the most prevalent male sexual disorder affecting up to 30 percent of sexually active men and is even more prevalent than erectile dysfunction (a $3.8 billion market in 2008). Currently, there are no FDA-approved products available for the treatment of this disorder in the United States.

NeuroHealing Pharmaceuticals, Inc. is a clinical stage private company developing innovative treatments for specialty indications based on repositioning neurologically active compounds. Clinical stage programs include: NH001 (phase II), a dopaminergic agent to help post traumaticbrain injury patients to emerge from a coma, vegetative or minimally consciousness state; NH004 (phase II), an anticholinergic agent in a convenient intra-orally muco-adhesive dissolving film to help treat motorneuron disease patients who suffer from sialorrhea (drooling); and NH02D, a neurologically active compound to treat premature ejaculation.
